Premenstrual dysphoric disorder (PMDD) is a cyclical mood condition tied to the hormonal changes of the menstrual cycle. It is far more than typical PMS. PMDD causes emotional and physical symptoms in the days before menstruation that can significantly disrupt relationships, work, and daily life and then resolve shortly after the period begins. At Vista Mental Health, we take PMDD seriously and provide comprehensive, individualized care to help you find lasting relief.
What is PMDD?
PMDD is a recognized psychiatric condition characterized by a predictable pattern of severe mood, behavioral, and physical symptoms that appear in the luteal phase of the menstrual cycle (typically the one to two weeks before menstruation) and resolve within a few days of the period beginning.
Symptoms that may occur in the luteal phase include:
- Marked mood swings, sudden tearfulness, or increased sensitivity to rejection
- Intense irritability, anger, or interpersonal conflict
- Depressed mood, feelings of hopelessness, or self-critical thoughts
- Significant anxiety, tension, or feeling on edge
- Difficulty concentrating
- Fatigue or low energy
- Changes in sleep — insomnia or sleeping too much
- Changes in appetite, food cravings, or overeating
- Physical symptoms such as breast tenderness, bloating, joint or muscle pain, or headaches
- A sense of feeling overwhelmed or out of control
To distinguish PMDD from other mood conditions, symptoms must be cyclical, must resolve after menstruation begins, and must cause meaningful distress or impairment in daily functioning.
What We Offer
Diagnosis & Assessment — Accurate diagnosis of PMDD requires careful attention to the timing and pattern of symptoms. We conduct thorough psychiatric evaluations and may recommend prospective symptom tracking across one to two cycles to confirm the diagnosis and rule out other conditions, including premenstrual exacerbation of underlying depression, anxiety, or bipolar disorder.
Medication Management — Effective, FDA-approved treatments are available for PMDD. Our psychiatrists evaluate each patient’s symptom pattern, cycle characteristics, and treatment history to develop a personalized medication plan. Treatment recommendations are based on current clinical evidence and tailored to your specific needs and goals.
Therapy — Psychotherapy can play an important role in PMDD care, both for managing the emotional symptoms directly and for addressing the relational and functional impact of the condition. Our therapists offer evidence-based approaches including Cognitive Behavioral Therapy (CBT) and mindfulness-based strategies, tailored to the cyclical nature of PMDD.
Integrative & Adjunctive Options — We take a whole-person approach to PMDD care, incorporating evidence-informed integrative strategies alongside conventional treatment. Nutritional considerations, exercise, sleep support, stress management, and select evidence-based supplements may be discussed as part of a comprehensive care plan.
Telehealth & Flexible Care — We offer telehealth appointments for both therapy and medication management when clinically appropriate. Many patients find virtual care especially convenient for ongoing PMDD management and cycle-based follow-up.
